Warmth and plenty of lube. Ahem.
If there are any sharp bits you are trying to pass it over, wrap with a layer of insulation tape, too. It will slide better over greased up insulation tape.

Thanks for that Gelandstrasse , took the complete cable off the bike, did it in the kitchen to get it a bit warmer.
Insulation tape , rubber bit in boiling water , washing up liquid and after a struggle it went on.
Found it easier with the boot inside out to push it on then flipped it back the correct way when done.
